Citizen Complaint Form

Use this form to notify the Department of Neighborhood Services and Inspections of problems in your neighborhood. You may also report problems by calling our office at (262) 653-4263.

Name, home address, and telephone number are required to submit this form. Your complaint will not be processed without contact information.

Note: Complaints NOT handled by the Department of Neighborhood Services and Inspections:

  • Trash, debris, or animal feces in yards (Contact the County Division of Health – 605-6700)
  • Junk cars/cars parked on front lawns (Contact the City Police Department – 605-5200)
  • Garbage pickup/drop off, recycling, public sidewalks, and approaches (Contact the City Department of Public Works – 653-4050)
  • Civil matters between tenants and landlords (Contact Consumer Protection – 1 (800) 422-7128)
